`POST /api/asclepius/environments/{task_id}/annotate`

A board-certified physician submits the §7 annotation for one trajectory
(run). Gated on ``portal_version == 'env'`` (PRD §12.9).

Legacy ``'v5'`` is accepted for one release — see the queue above. It is never
written: an env annotation lands in ``env_runs``, which has no portal_version
column, so there is no way for the legacy string to be mistaken later for a
longitudinal submission.
