Diagnostic: run the REAL multimodal generation pipeline once and report what
happened — the accepted count and the exact per-reason DROP breakdown — so an
operator can see precisely why V3 is (or isn't) getting structured cases instead
of guessing. Synthetic data only (no PHI), so it is available to any signed-in
evaluator. Accepted cases are inserted into the queue (this doubles as a manual
'force a case' button).
