Close a request as ``fulfilled`` or ``withdrawn``.

Two reasons and no free text, because the reason is read by a person
scanning a list rather than by whoever wrote it. ``fulfilled`` says we got
what we asked for; ``withdrawn`` says we stopped asking, and a partner
reading the portal should not have to guess which.
