Everything the five scope pickers need, in one call.

``specialties`` / ``versions`` are what the old two-filter surface returned
and are unchanged. ``cases`` and ``physicians`` are new (PRD §2.1): the Case
scope types or pastes an id and wants a typeahead behind it, and the
Physician scope picks a PERSON BY NAME while the bundle it produces carries
only ``annotator_id_hashed``. Both halves come from one row so a caller
cannot cross them.
