Get a single alive token by address.

- TokenWithLevel: Alive token for API responses. Level fields are LIVE as of migration 061. ``level`` is a high-water-mark ratchet column maintained by the fee worker (``max(level, floor(total_fees_sol / 0.5))``; was generated until migration 092); the other two are computed in dashboard.py from ``total_fees_sol``, ``level``, and ``LEVEL_FEE_THRESHOLD_SOL``. Frontend renders these as a level number + progress bar to the next level — the gamification surface for "earn fees → earn compute."
