Get AI-powered treatment recommendations for a patient.

Gathers evidence from:
- Patient medical history and context
- PubMed literature
- ClinicalTrials.gov
- Open Targets gene-disease associations

Returns evidence-based treatment suggestions with citations.

## Request body

- TreatmentRecommendationRequest: Request for treatment recommendations.
  - `patient_id` integer, required: Patient ID
  - `specific_question` string, nullable: Specific question to focus on (optional)
  - `include_trials` boolean: Include clinical trial matches
  - `include_literature` boolean: Include PubMed literature
