`POST /clinical-ai/chat`

## Request

Interactive clinical Q&A with patient context.

Supports multi-turn conversation about a patient's case,
with full access to patient history and clinical evidence.

## Request body

- ClinicalChatRequest: Request for clinical Q&A chat.
  - `patient_id` integer, required: Patient ID for context
  - `message` string, required: User question or message
  - `conversation_history` object[], nullable: Previous messages in conversation
