Feedback is stored for model improvement and learning.

  - `action` string, required: Feedback action: accepted, modified, rejected, data_correction
  - `selected_treatment` string, nullable: Treatment chosen by clinician
  - `rationale` string, nullable: Clinical reasoning for the decision
  - `modifications` object, nullable: Any modifications to AI recommendations
