Per-entity label distribution + agreement metrics.

Headline view for binary-mode rankings. Returns true_count /
false_count, agreement_rate (max(true,false)/total), Wilson 95% CI
on P(true), and a controversy index that peaks at 50/50 splits.

`sort` modes:
  - controversial (default) — highest disagreement first; what
    creators want to see for triage
  - consensus — clearest signals first (max agreement)
  - recent — newest entities first
  - name — alphabetical
