Pairwise-κ ceiling estimate from qualification attempts.

The ceiling is the upper bound any judge / RM can credibly hit on
this domain — no model can exceed human inter-rater agreement
without overfitting noise. Compute by having SMEs jointly rate the
qualification gold-standard items, then taking the mean of pairwise
Cohen κ across all SME pairs that have at least
`min_overlapping_items` jointly-rated items.
