局部更新。404 而不是创建：PATCH 一个不存在的技能是名字写错了。

- SkillPatch: 只改传来的字段。理由同 AgentPatch：PUT 是整体替换，一个漏掉 instructions 的 PUT 会把技能的正文清空。 `tools_add`/`tools_remove` 是增量语义（同 AgentPatch）：`tools=[...]` 整表替换， 想"加/删一个"用增量，免得把没读到的其它项挤掉。幂等。
