Single JSON-RPC entry point for ``initialize`` / ``tools/list`` /
``tools/call``. Auth = the existing app_token Bearer, tenant-scoped via
``current_tenant_ctx`` (deny-by-default). A malformed body returns a proper
JSON-RPC parse/invalid-request error; a tool failure returns isError, never
a 500.
